Untreated gout attacks can last days and sometimes longer, while effective treatment can shorten symptoms for many people. Severity, timing of treatment and whether the diagnosis is actually gout all matter.
When the pattern is unusual
A first episode, prolonged severe symptoms, fever, feeling very unwell or diagnostic uncertainty deserves medical assessment rather than assuming it is simply a long gout attack.
