Treat-to-target urate lowering

NICE recommends adjusting ULT against repeated serum urate measurements, with a usual target below 360 µmol/L and a lower target considered in some people with more severe gout.

DIAGNOSIS

One urate result is not the whole diagnosis

Serum urate can be lower during an acute flare. When gout is strongly suspected despite a result below 360 µmol/L, NICE advises repeating the measurement after the flare has settled.

Five questions before believing it.

01Who was studied? People with gout, people with high urate, animals or cells?
02What was measured? Urate, flares, pain, inflammation markers or something indirect?
03Was it an intervention? An association does not prove the thing caused the outcome.
04How large was the effect? Statistical significance is not the same as practical importance.
05Does guidance agree? One new paper rarely overturns established care on its own.
